Obtain the application on-line below or at the Arlington Branch Library. Complete the form and submit online at arlingtonvillage699@gmail.com or drop in the utility deposit box at the Municipal Building. Do not return the completed form to the Library.
2) Application form includes the following:
a. Name / Address change
b. Selling House / Moving Out
c. Buying House / Moving In
